How do I know if my clutch is going?

Yo, I'm not that mechanically adept and I'm wondering whether you guys could give me input into whether my clutch on my facelift Sportivo needs replacement. Whenever I take off the revs shoot pretty high but the acceleration is way slower than it used to be. Prior to the slow acceleration the clutch used to omit a slight burning smell, but it doesn't do that as often as it used to. I don't think the clutch has been changed on my car and it's been 137000km. Does anyone know any sure fire ways of telling whether my clutch is slipping and needs replacing?? Also, sometimes when I try to accelerate it revs to around 3-4km, hesitates and doesn't go further.

Handbrake on tight, left foot on clutch pedal, right foot hard on brake pedal, put into fifth gear and let your left foot off of clutch. If the clutch is stuffed, the car will rev its tits off and go nowhere.
